How much intentional work does it actually take?

I know these are not miracle drugs and that along with the medication you need to “do your part” (getting enough protein/strength training). But for those who have lost the weight, how much mental work did it really take? Like do you still constantly have to worry about what/how much you eat? I don’t love the idea of counting calories while on the medication because I can do that without the medication and lose weight. I was a chronic calorie counter before and I’m really trying to break that habit and have a better/ more intuitive relationship with food. Calorie counting wasn’t sustainable for me and became a vicious cycle of losing the weight and regaining it. But I also know that I won’t drop weight accidentally just by being on the medication. I’m on my 4th week of the .25 dose and haven’t noticed any side effects, nor have I lost weight. (I know it’s not even the therapeutic dose yet). I guess it’s just mind boggling to me that I might actually be able to lose weight without calorie counting! So again my question, how much intentional, active work does it take to lose weight while using this medication?
