Allo transplant after failed CAR-T treatment

Post Content

Hi everyone - need a little help please. My mom was diagnosed with stage 4 DLBCL in May 2025. She started with the standard RCHOP regimen and then quickly changed to DAR-EPOCH once her oncologist found that she had double hit lymphoma. After three rounds of chemo, her PET scan showed that her cancer was refractory and continuing to grow. They then changed her regimen again to POLAR-GEM-OX as a bridge to CAR-T. The chemo worked at getting her close to remission before starting the CAR-T treatment. She had all of the possible side effects (severe ICANS, severe CRS, etc.) and wound up in the hospital for three months. Her oncologists were sure that the CAR T had done its job, but the follow-up pet scan in January showed that while CAR T did help reduce the cancer burden, it ultimately failed. She is going to start her second round of POLA-ICE in a few days. They’re using it as a bridge to getting an allo transplant. Her oncologists are working on finding her a match and will proceed depending on how her PET scans look in about a month. Has anyone had any experience with having an allo transplant after failed CAR-T treatment? Any advice would be helpful. Thank you!
